APPENDIX Appendix A – New Nursing Homes and Recent Changes in Facility Name and/or Number of Licensed Beds 47 PREFACE This is the twentieth edition of the Delaware Nursing Home Utilization Statistical Report. The source for all information for this document is monthly utilization statistical reports by Delaware nursing homes submitted to the Bureau of Health Resources Management. The data in this report does not include Intermediate Care Facilities for the Mentally Retarded (ICFMR). ICFMR facilities provide active treatment services to people with metal retardation and other related conditions causing adaptive behavior similar to that of the mentally retarded person and is manifested before the person reaches the age of 22. Note: * Percentages may appear as less than 100% due to rounding. * See appendix A for recent changes in facility names and/or number of licensed beds. SUMMARY This report presents characteristics of Delaware's nursing homes and their patients. While focusing on information from 2005, the Summary presents various data from 1995 through 2005. Information generally relates to the following: (1) Facility characteristics (number of homes and beds by ownership) (2) Utilization data (patient days, occupancy, admissions and discharges) (3) Patient characteristics (age, sex, source of payment) Facility Characteristics The number of nursing homes in the state increased to 46 from 45 in 2004, but decreased by two from 48 in 1995. The number of licensed beds increased from 4,707 in 2004 to 4,822 in 2005. Of the total beds, approximately 83.6% were privately owned in 1995, increasing to 86.5% in 2005. The percentage of beds in public facilities decreased correspondingly from 16.4% to 13.5%. By county, there was a 1.9% decrease in New Castle, a 3.9% decrease in Sussex and a 0.7% decrease in Kent. The public facilities experienced an increase of 2.3%. Private facilities in New Castle decreased from 93.0% to 87.2%. Private facilities in Kent County decreased from 93.1% to 92.7%. Private facilities in Sussex County decreased from 92.2% to 88.8%. The occupancy rate in public facilities increased from 55.5% to 57.0%. When based upon the number of beds for which operating funds were budgeted, the occupancy rate in public facilities was considerably higher. Admissions to private facilities increased by 64.9% while admissions to public facilities decreased by 36.4%. As in the past, in 2005 the largest percentage of admissions (37.8%) was persons in the 75-84 age group. Admissions in the 85+ age group accounted for 33.2%. As mentioned above, the largest number of admissions to Delaware nursing homes was for individuals aged 75-84 (37.8%) and for individuals age 85 and older (33.2%). According to the Delaware Population Consortium, Populations Projections Series, dated October 23, 2007, in 2005 there were 40,665 Delawareans between ages 75 and 84 and 14,084 Delawareans age 85 and up. Admissions to Delaware nursing homes for individuals within these age groups were 3,368 and 2,956 respectively. Keeping in mind that not all persons admitted to Delaware nursing homes were Delawareans – some came from other states – it is probably fair to estimate that about 8 percent of those in the 74-85 age group and 21 percent of those age 85 and older were at some point in time admitted to a nursing home. Clearly, the likelihood of being admitted to a nursing home increases with age. In public facilities, 37.2% of admissions were from hospitals during 2005 and 40.0% from patients' or relatives' homes. Age of patient admission varies greatly by race. Blacks tend to enter nursing homes in Delaware at a much younger age than whites. Of the black admissions in 2005, 46.8% were less than 75 years of age, compared to 25.0% for whites. The following chart identifies the percentage of patients admitted to Delaware nursing homes by race within specific age groups. Example: 9.4 percent of all white admissions were under the age of 65 and 25.7 percent of all black admissions were under the age of 65. Throughout all ages the number of white patient discharges is much higher than that for blacks and others combined. The following chart identifies the percent of discharges by race within specific age groups. The proportion of patients discharged from private facilities to a patients’ own or to a relatives’ home increased from 42.0% in 2004 to 43.7% in 2005. During the same period, patients discharged due to death decreased from 18.2% to 17.9%, while discharges to acute care hospitals decreased from 30.4% to 28.5%. Comparing public and private facilities, patients in public facilities are much less likely to be discharged to home or hospital and much more likely to be discharged due to death (see Table 8.1). In private facilities, while approximately 46.1% of the residents in 1995 had Medicaid as the source of payment, 54.3% of the residents in 2005 had Medicaid as the source of payment and they represented a growing portion of the Medicaid patient days (see graph on page 10). About 27.8% of patients in private facilities were self-pay. In public facilities, on the other hand, 77.8% of the residents had Medicaid as their source of payment in 2005 while only 5.4% were self-pay. Medicare is not a major payer of nursing home care. In 2005, Medicare was the source of payment for 14.2% of the patients in private facilities and 2.0% in public facilities. However, the percent of Medicare patients has changed considerably since 1995 when the percentages of patients with Medicare as a source of payment were 7.4% for private facilities and 2.8% for public facilities. Medicaid patient days in public facilities for the period of 2004 through 2005 were reduced slightly by 0.16%%. Medicaid patient days for private facilities were reduced 6.3% over the same period of time.
