The Department of Health and Social Services (DHSS) and the Delaware Health Care Commission (DHCC) has been charged with developing a pathway to affordable healthcare for all Delawareans. In an effort to increase access to care, The Commission is offering one time funding to help medical practices, Federally Qualified Health Centers (FQHCs), Behavioral Health practices, Accountable Care Organizations (as defined by Medicare MSSP), hospitals and clinically integrated networks grow their capacity to integrate data, improve the coordination of patient care and increase their readiness to integrate into a total cost of care or other Alternative Payment Model (AMP).

You and/or the organization you work has identified your practice as qualificed, please leverage the funds we are offering through the State Innovation Model (SIM) grant. We strongly encourage collaboration/partnership between providers who, without these funds, cannot complete their proposed project(s).

Applications are due no later than August 30, 2018. Should you leverage this opportunity; applications will be evaluated and considered as they are received. The DHSS and DHCC are prepared to award up to 10 applicants in amounts ranging from $25,000 through $250,000.

  • Organizations information
  • Narrative project description
  • Project work plan
  • Project budget
  • A copy of your organizations medical license (only if applicable)
  • Organizations Delaware business license

DHSS/DHCC will hold a question and answer session via conference call on Thursday, August 9, 2018 at 2:00 PM (see call-in information below). Please submit final applications and/or questions by email to DHCC@delaware.gov

Eligible Applicants

  • Primary care providers, behavioral health providers, ACOs, hospitals, FQHCs and clinically integrated networks.
  • Licensed in the State of Delaware.
  • Can apply for:
    • Small Project (up to $50,000)
    • Large Project (up to $250,000)
  • May apply for multiple projects.
  • Final date to submit applications: Aug. 30, 2018
  • Q&A conference call session at 2 p.m. Aug. 9 or email DHCC@delaware.gov

Eligible Projects

  • Data Integration: Enhance the applicant's data integration, clinical informatics or population-based analytics capabilities.
    • Examples of a Data Integration project would be:
      • Data exchange infrastructure and analytics projects or support.
      • Data warehousing and reporting capacity.
      • Development of data-sharing agreements.
  • Improved Coordination of Patient Care: Enhance the applicant's clinical integration.
    • Examples of the Improved Coordination of Patient Care would include:
      • Conducting data analytics and developing care guidelines for primary care based system of complex care management for high-risk populations.
      • Implementing improvements in care transitions, such as new business processes, mutual agreements with partner providers, etc.
      • Implementing a practice support call center.
  • Increased Readiness to Integrate into an Accountable Care Organization (ACO) or operate through an Alternative Payment Method (APM): A project will develop, expand or enhance the applicant's shared governance structures and organizational integration strategies, linking the applicants with ACO leadership and across the continuum of care with providers.
    • Examples of Readiness to Integrate into an ACO or operated through an Alternative Payment Method would include:
      • Model costs of care in preparation for participation in value-based payment arrangements with multiple payers.
