APPENDIX Appendix A – New Nursing Homes and Recent Changes in Facility Name and/or Number of Licensed Beds 47 PREFACE This is the nineteenth edition of the Delaware Nursing Home Utilization Statistical Report. The source for all information for this document is monthly utilization statistical reports by Delaware nursing home submitted to the Bureau of Health Resources Management. The data in this report does not include Intermediate Care Facilities for the Mentally Retarded (ICFMR). ICFMR facilities provide active treatment services to people with mental retardation and other related conditions causing adaptive behavior similar to that of the mentally retarded person and is manifested before the person reaches the age of 22. SUMMARY This report presents characteristics of Delaware's nursing homes and their patients. While focusing on information from 2004, the Summary presents various data from 1994 through 2004. Information generally relates to the following: (1) Facility characteristics (number of homes and beds by ownership) (2) Utilization data (patient days, occupancy, admissions and discharges) (3) Patient characteristics (age, sex, source of payment) Facility Characteristics The number of nursing homes in the state remained the same at 45 from 2002 to 2004, but decreased by four from 49 in 1994. The number of licensed beds decreased from 4,756 in 2003 to 4,707 in 2004. Of the total beds, approximately 83.6% were privately owned in 1994, increasing to 86.1% in 2004. The percentage of beds in public facilities decreased correspondingly from 16.4% to 13.9%. By county, there was a 3.0% increase in New Castle, a 0.6% increase in Sussex and a 0.8% decrease in Kent. The public facilities experienced a decrease of 7.9%. Private facilities in New Castle increased from 89.5% to 93.0%. Private facilities in Kent County increased from 92.5% to 93.1%. Private facilities in Sussex County increased from 91.0% to 92.2%. The occupancy rate in public facilities decreased from 60.2% to 55.5%. When based upon the number of beds for which operating funds were budgeted, the occupancy rate in public facilities was considerably higher. Admissions to private facilities increased by 71.2% while admissions to public facilities decreased by 60.0%. As in the past, in 2004 the largest percentage of admissions (39.3%) was persons in the 75-84 age group. Admissions in the 85+ age group accounted for 34.2%. In public facilities, 22.1% of admissions were from hospitals during 2004 and 42.6% from patients' or relatives' homes. Age of patient admission varies greatly by race. Blacks tend to enter nursing homes in Delaware at a much younger age than whites. Of the black admissions in 2004, 42.8% were less than 75 years of age, compared to 23.0% for whites. The following chart identifies the percentage of patients admitted to Delaware nursing homes by race within specific age groups. Example: 8.2 percent of all white admissions were under the age of 65 and 20.7 percent of all black admissions were under the age of 65. Throughout all ages the number of white patient discharges is much higher than that for blacks and others combined. The following chart identifies the percent of discharges by race within specific age groups. Example: 64 percent of all white patients discharged were under the age of 65 and 32 percent of all black patients discharged were under the age of 65. The proportion of patients discharged from private facilities to the patients’ own or relatives' homes decreased from 43.5% in 2003 to 42.0% in 2004. During the same period, patients discharged due to death decreased from 20.6% to 18.2%, while discharges to acute care hospitals increased from 27.4% to 30.4%. Comparing public and private facilities, patients in public facilities are much less likely to be discharged to home or hospital and much more likely to be discharged due to death (see Table 8.1). In private facilities, while approximately 45.4% of the residents in 1994 had Medicaid as the source of payment, 54.7% of the residents in 2004 had Medicaid as the source of payment and they represented a growing portion of the Medicaid patient days (see graph on page 10). About 29.7% of patients in private facilities were self-pay. In public facilities, on the other hand, 79.7% of the residents had Medicaid as their source of payment in 2004 while only 3.2% were self-pay. Medicare is not a major payor of nursing home care. In 2004, Medicare was the source of payment for 13.1% of the patients in private facilities and 2.1% in public facilities. However, the percent of Medicare patients has changed considerably since 1994 when the percentages of patients with Medicare as a source of payment were 6.7% for private facilities and 2.6% for public facilities. Medicaid patient days, in public facilities for the period of 2003 through 2004 were reduced by 11.6%. Medicaid patient days for private facilities increased 6.3% over the same period of time.
