General Information
Two Medicaid Home and Community-Based Waivers operated by the Division of Services for Aging and Adults with Physical Disabilities (DSAAPD) were discontinued effective 12-1-10. On that date, the services included in the two discontinued waivers were incorporated into an amended Medicaid Waiver for the Elderly and Disabled. This page provides copies of materials related to these waiver programs for archival purposes.
