REQUEST FOR SERVICES This is a REQUEST FOR SERVICES notice #PSC 867 for SUPPORTED EMPLOYMENT, and/or TRADITIONAL COMPETITIVE JOB PLACEMENT SERVICES, JOB COACHING, VOCATIONAL TRAINING SERVICES, VOCATIONAL EVALUATIONS, WORK ADJUSTMENT TRAINING, JOB READINESS TRAINING, COMPUTER TRAINING SKILLS, TRIAL WORK EXPERIENCES, PSYCHOLOGICAL EVALUATIONS, NEUROPSYCHOLOGICAL EVALUATIONS, MENTAL HEALTH COUNSELING, PSYCHIATRIC CONSULTS, MEDICAL CONSULTS, EYE CONSULTS, LOW VISION THERAPIST, and SMALL BUSINESS CONSULTANT TECHNICAL SERVICES for the Division of the Visually Impaired (DVI), Delaware Health and Social Services (DHSS), 1901 N. DuPont Highway, New Castle, DE. 19720. The DVI is seeking to contract with providers who have the interest and ability to provide job placement services for persons with visual disabilities and other related conditions who are eligible to receive such services under the auspices of DVI. The type of job placement services will depend on individual need for intensive job supports. In addition to the use of objective provider-selection criteria, it is anticipated that the personal and/or representative choice of service recipients will be a significant factor in the ultimate selection of service providers. Community Rehabilitation Programs and other entities that wish to be considered to provide the above referenced job placement services must: 1. Attend a mandatory REQUEST FOR SERVICES meeting on Tuesday June 9, 2009 from 2:00 pm – 3:00 pm. The meeting will be held at the Division for the Visually Impaired, 1901 N. DuPont Highway, Biggs Bldg Door # 3, 202 Calle Carmen Nazario in New Castle, DE. 2. Be willing to accept reimbursement for services at accomplished milestones based on rates that are negotiated between DVI and the service provider. 3. Submit a Letter of Interest regarding this REQUEST FOR SERVICES prior to the meeting date of Tuesday June 9, 2009, or at the conclusion of the meeting for those in attendance only, or no later then 4:30 pm on June 26, 2009.
