Delaware 911 Form
The Delaware State Police has developed a 911 form to assist persons with special needs in emergency situations.
The form is designed to collect information that will help support persons with special needs when 9-1-1 is called. Individuals are encouraged to fill out the form and send it to the State Police. Once the form is received, information about a person's special needs is stored in the 911 system and will be immediately available to first responders in emergency situations. Submission of the form is voluntary and all information is kept confidential
