Hospital Discharge Support Referral
General Information
The Delaware Aging and Disability Resource Center (ADRC) is available to assist discharge planners in Delaware's acute care hospitals. The ADRC can help by providing options counseling and service enrollment support to older persons and adults with physical disabilities as they transition from the hospital to their homes or to other care settings.
Options counseling enables individuals and families to evaluate, discuss, identify, and/or clarify long term care options, needs and/or preferences. Service enrollment support helps individuals and families locate and/or access long term care services.
How to Make a Hospital Discharge Support Referral
Follow the "Make a Referral " link below to access the ADRC Secure Referral System. On the referral screen, select "Referral Source", then hit the "Proceed" button. Please fill in as much information as possible and submit the referral. A printable copy of the referral will be displayed upon submission. An ADRC Support Services contractor will respond to your referral.
