We have set ambitious goals for improving health care for\n\n\n\n  Delawareans and improving the sustainability of our health care system, and are supported by a broad set of stakeholders who are\n\n\n\n  committed to making real and lasting change &#8211; including physicians, hospitals, other providers, insurers, businesses, consumer advocates\n\n\n\n  and our education and research community.\n\n\n\n<\/p>\n\n\n\n<h2>\n\n\n\n  Why Delaware is developing a State Health Care Innovation Plan\n\n\n\n<\/h2>\n\n\n\n<p>\n\n\n\n  The goal is the &#8220;Triple Aim&#8221; &#8211; improving the health of Delawareans, improving the patient experience of care, and reducing health care\n\n\n\n  costs. Through the SIM initiative, Delaware is planning to build upon a strong local foundation for innovation in order to achieve this\n\n\n\n  through a system-level transformative healthcare plan that can serve as a scalable model for the nation. Our aspiration is to develop and\n\n\n\n  implement a plan for broad-based health system transformation, including new payment and delivery models, which will achieve the Triple\n\n\n\n  Aim.\n\n\n\n<\/p>\n\n\n\n<p>\n\n\n\n  Delaware has been awarded a &#8220;Design&#8221; grant through the CMMI SIM initiative to provide funding as we develop our State Health Care\n\n\n\n  Innovation Plan. We will be applying for a &#8220;Testing&#8221; grant &#8211; which could provide significant funding to support the implementation of our\n\n\n\n  plan.\n\n\n\n<\/p>\n\n\n\n<p>\n\n\n\n  State Health Care Innovation Plans must be Governor-led, multi-payer, and focused on achieving the Triple Aim of improving health,\n\n\n\n  improving the patient experience of care, and reducing health care costs.\n\n\n\n<\/p>\n\n\n\n<h2>\n\n\n\n  Our approach\n\n\n\n<\/h2>\n\n\n\n<p>\n\n\n\n  We are organizing our health transformation approach in six workstreams:\n\n\n\n<\/p>\n\n\n\n<ul>\n\n\n\n  <li>Delivery system\n\n\n\n  <\/li>\n\n\n\n  <li>Population health\n\n\n\n  <\/li>\n\n\n\n  <li>Payment model\n\n\n\n  <\/li>\n\n\n\n  <li>Data and analytics\n\n\n\n  <\/li>\n\n\n\n  <li>Workforce\n\n\n\n  <\/li>\n\n\n\n  <li>Policy\n\n\n\n  <\/li>\n\n\n\n<\/ul>\n\n\n\n<p>\n\n\n\n  Our guiding principles:\n\n\n\n<\/p>\n\n\n\n<ul>\n\n\n\n  <li>Develop a health care transformation strategy that is multi-payer and multi-stakeholder and focuses on achieving the &#8220;Triple Aim&#8221;\n\n\n\n  <\/li>\n\n\n\n  <li>Be one of the leading states in innovation and impact\n\n\n\n  <\/li>\n\n\n\n  <li>Achieve measurable results in three years through practical implementable goals\n\n\n\n  <\/li>\n\n\n\n  <li>Meet the near term objective of developing the State Health Care Innovation Plan while focusing on the primary goal of transforming\n\n\n\n  Delaware&#8217;s health care\n\n\n\n  <\/li>\n\n\n\n  <li>Focus on the best interests of all Delawareans and respect the voice of consumers\n\n\n\n  <\/li>\n\n\n\n  <li>Have no &#8220;sacred cows&#8221;\n\n\n\n  <\/li>\n\n\n\n  <li>Learn and make use of best practice examples from other states and countries where possible\n\n\n\n  <\/li>\n\n\n\n  <li>Focus on getting to a practical plan\n\n\n\n  <\/li>\n\n\n\n<\/ul>\n\n\n\n<h2>\n\n\n\n  Delaware Center for Health Innovation Board of Directors Meetings\n\n\n\n<\/h2><br>\n\n\n\n<ul>\n\n\n\n  <li>\n\n\n\n    <strong>Meeting Notices, Agendas, Minutes<\/strong>\n\n\n\n  <\/li>\n\n\n\n  <li style=\"list-style: none\">\n\n\n\n    <br>\n\n\n\n    &#8211; Visit the State Public Meeting Calendar: <a href=\"http:\/\/egov.delaware.gov\/pmc\/\">egov.delaware.gov\/pmc\/<\/a><br>\n\n\n\n    &#8211; Click &#8220;List by Agency&#8221;, and scroll to the &#8220;Delaware Health Care Commission&#8221; (DHCC).<br>\n\n\n\n    &#8211; All DHCC meetings are listed by date.
