Pursuant to Chapter93 Health Planning and\n\n\n\n  Resources Management, \ufffd9305, application fees shall be determined from the following table:\n\n\n\n<\/p><br>\n\n\n\n<table class=\"table table-condensed table-hover\">\n\n\n\n  <tbody><tr class=\"info\">\n\n\n\n    <th>\n\n\n\n      Capital Expenditures\n\n\n\n    <\/th>\n\n\n\n    <th>\n\n\n\n      Application Fee\n\n\n\n    <\/th>\n\n\n\n  <\/tr>\n\n\n\n  <tr>\n\n\n\n    <td>\n\n\n\n      Less than $ 500,000\n\n\n\n    <\/td>\n\n\n\n    <td>\n\n\n\n      $100\n\n\n\n    <\/td>\n\n\n\n  <\/tr>\n\n\n\n  <tr>\n\n\n\n    <td>\n\n\n\n      $500,000 to $999,999\n\n\n\n    <\/td>\n\n\n\n    <td>\n\n\n\n      $750\n\n\n\n    <\/td>\n\n\n\n  <\/tr>\n\n\n\n  <tr>\n\n\n\n    <td>\n\n\n\n      $1,000,000 to $4,999,999\n\n\n\n    <\/td>\n\n\n\n    <td>\n\n\n\n      $3000\n\n\n\n    <\/td>\n\n\n\n  <\/tr>\n\n\n\n  <tr>\n\n\n\n    <td>\n\n\n\n      $5,000,000 to $9,999,999\n\n\n\n    <\/td>\n\n\n\n    <td>\n\n\n\n      $7500\n\n\n\n    <\/td>\n\n\n\n  <\/tr>\n\n\n\n  <tr>\n\n\n\n    <td>\n\n\n\n      $10,000,000 and over\n\n\n\n    <\/td>\n\n\n\n    <td>\n\n\n\n      $10,000\n\n\n\n    <\/td>\n\n\n\n  <\/tr>\n\n\n\n<\/tbody><\/table><br>\n\n\n\n<br>\n\n\n\n<p>\n\n\n\n  Filing fees shall be due 30 days after the date of notification of the beginning of review as covered under \ufffd9305 subdivision (4). This\n\n\n\n  due date may be extended up to 10 additional days at the discretion of the Bureau. Applications for which filing fees have not been paid\n\n\n\n  within this time frame shall be considered to be withdrawn.
