Below are the Spay & Neuter Program forms for use by for-profit surgery providers and veterinarians and non-profit animal clinics, shelters and rescue organizations.

FOR-PROFIT SURGERY PROVIDER AND VETERINARIAN FORMS

• Spay & Neuter Program Monthly Rabies Verification and Surcharge Remittance Report
  The Spay & Neuter Program Monthly Rabies Verification and Surcharge Remittance Report must be submitted with rabies inoculation surcharges. Veterinarians who administer rabies inoculations in Delaware must collect a $3 surcharge for each vaccine and forward collected surcharges on a monthly basis, together with all applicable rabies inoculation verifications, to the Division of Public Health.
• Spay & Neuter Program Surgery Provider Agreement
  Veterinarians who do not participate in the Spay & Neuter Program must submit the Spay & Neuter Program Surgery Provider Agreement to the Spay & Neuter Program coordinator to become a participating surgery provider.
• Spay & Neuter Program For-Profit Provider Invoice
  Participating Spay & Neuter Program for-profit surgery providers should submit a Delaware Spay & Neuter Program For-Profit Provider Invoice for any program surgeries performed and a Complications Invoice, if applicable, on a monthly basis to the Spay & Neuter Program coordinator. These forms must be accompanied by the Income Eligible Application and Application Checklist and Surgery Information forms for each pet sterilized. NON-PROFIT ANIMAL CLINIC, SHELTER AND RESCUE FORMS

• Spay & Neuter Program Monthly Rabies Verification and Surcharge Remittance Report

  Non-profit shelters and clinics who wish to perform surgeries for the Spay & Neuter Program must submit a Spay & Neuter Program Surgery Provider Agreement to the Spay & Neuter Program coordinator to become a participating non-profit surgery provider.
• Spay & Neuter Program Non-Profit Provider Invoice
  Participating Spay & Neuter Program non-profit surgery providers should submit a Delaware Spay & Neuter Program Non-Profit Provider Invoice for any program surgeries performed and a Complications Invoice, if applicable, on a monthly basis to the Spay & Neuter Program coordinator. These forms must be accompanied by the Spay & Neuter Program Non-Profit Application for Sterilization of a Stray Cat or Dog forms for each animal sterilized. • Spay & Neuter Program Complications Invoice
• Spay & Neuter Program 501(c)(3) Participant Agreement
  Non-profit rescues or shelters who do not participate in the Spay & Neuter program must submit the Spay & Neuter Program 501(c)(3) Participant Agreement to the Spay & Neuter Program coordinator.
