The State of Delaware provides 100% of the funding for this program, which is administered by the Division of Medicaid &#038; Medical Assistance (DMMA). As State funds are limited, ensuring the widest coverage requires that all third party resources (Medicare, Medicaid, VA, private insurance, etc.) must be considered before CRDP funds are used.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h2>Eligibility<\/h2>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>The applicant must meet certain medical and financial criteria to be eligible for support. Medically, the individual must be diagnosed with End Stage Renal Disease (ESRD), receive dialysis or have had a renal transplant. Financially, the individual&#8217;s gross countable income must be below <a class=\"anchors_link\" href=\"fpl.html#300fpl\">300% of the Federal Poverty Level<\/a>. Applicants with a legally married spouse will be considered as a household of two unless the couple is separated and maintains two separate residences for at least 12 months prior to application.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>Applicants who have Medicare A or B must apply for <a href=\"https:\/\/www.medicare.gov\/part-d\/index.html\">Medicare Part D Prescription Drug Coverage<\/a> and if eligible, for <a href=\"http:\/\/www.ssa.gov\/prescriptionhelp\/\">Social Security&#8217;s Extra Help Program<\/a>, unless they have other insurance that provides equivalent prescription benefits.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h2>Services Covered<\/h2>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>The Chronic Renal Disease Program can provide payment for the unreimbursed cost of medications (prescription and over-the-counter) and nutritional supplements, including the cost of Medicare Part D prescription drug coverage. In addition, transportation may be provided to and from the dialysis unit, transplant center or possible related medical appointments.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h2>Services Not Covered<\/h2>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>The program cannot pay for insurance premiums (except for Medicare Part D premiums) or any medical services other than those described above.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h2>How To Contact Us<\/h2>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>Our office is located in the <a href=\"\/main\/maps\/dsscmap\/milford.htm\">Milford State Service Center<\/a>. To start the referral process, the applicant must complete a CRDP application. You may call our office and ask for an application to be mailed or faxed to you. We can be contacted by calling the Delaware Help Line at <strong>1-800-464-4357<\/strong> and asking to be transferred to the Chronic Renal Disease Program, or you may call <strong>1-302-424-7180<\/strong>.
