To gauge effectiveness in meeting this mission, we collect data \n\n\n\n\n\non and monitor many health outcomes and continually engage in activities to improve these health outcomes. This \n\n\n\n\n\nweb page provides access to the DPH accreditation report, some of the health outcome indicators DPH monitors, \n\n\n\n\n\nthe DPH strategic plan, the State Health Assessment (SHA) and State Health Improvement Plan (SHIP).<\/p>\n\n\n\n\n\n<br>\n\n\n\n\n\n\n\n\n\n\n\n<h2>Public Health Accreditation<\/h2>\n\n\n\n\n\n\n\n\n\n\n\n<p>On March 8, 2016 the DPH was awarded national accreditation by the national Public Health Accreditation\n\n\n\n\n\nBoard (PHAB). The <a href=\"\/wp-content\/uploads\/sites\/10\/dph\/pdf\/phabsitevisitreport.pdf\"><strong>PHAB Site Visit Report<\/strong><\/a> provides an overview of \n\n\n\n\n\nhow DPH met the national public health standards.<\/p>\n\n\n\n\n\n<br>\n\n\n\n\n\n\n\n\n\n\n\n<h2>Health Outcome Indicators<\/h2>\n\n\n\n\n\n\n\n\n\n\n\n<p>As DPH continues its journey to improve public health, we gauge progress by collecting data on and monitoring many\n\n\n\n\n\nhealth outcomes. While not all inclusive, these <a href=\"toplevelmap.html\"><strong>health outcome indicators<\/strong><\/a> represent our current strategic focus.<\/p>\n\n\n\n\n\n<br>\n\n\n\n\n\n\n\n\n\n\n\n<h2>Division of Public Health 2019-2023 Strategic Plan<\/h2>\n\n\n\n\n\n\n\n\n\n\n\n<p>The <a href=\"\/wp-content\/uploads\/sites\/10\/dph\/pdf\/dphstrategicplan.pdf\"><strong>DPH strategic plan<\/strong><\/a> provides a clear path for the division to be a vital part of the state public\n\n\n\n\n\nhealth system. This dynamic plan provides focus for public health staff on important health priorities and is\n\n\n\n\n\naligned with the State Health Assessment (SHA) and State Health Improvement Plan (SHIP).<\/p>\n\n\n\n\n\n<br>\n\n\n\n\n\n\n\n\n\n\n\n<h2>State Health Assessment and State Health Improvement Plan<\/h2>\n\n\n\n\n\n\n\n\n\n\n\n<p>The <a href=\"\/wp-content\/uploads\/sites\/10\/dph\/pdf\/shaship.pdf\"><strong>State Health Assessment (SHA) and State Health Improvement Plan (SHIP)<\/strong><\/a> represent collective efforts of public and private groups as well as the residents of Delaware, to reveal health challenges and implement \n\n\n\n\n\nstatewide activities to address those challenges. The University of Delaware Partnership for Healthy Communities (PHC\/UD) and the Delaware Academy of Medicine\/Delaware Public Health Association (Academy\/DPHA), of which DPH is a partner, manages both the SHA and SHIP process in Delaware.<\/p>\n\n\n\n\n\n<p>In 2018 the <a href=\"\/wp-content\/uploads\/sites\/10\/dph\/pdf\/shipaandrsummary2017.pdf\"><strong>Summary of Delaware State Health Needs Assessment and Delaware State Health Improvement Plan Recommendations Report<\/strong><\/a> was published.<\/p>\n\n\n\n\n\n<br>\n\n\n\n\n\n<hr>\n\n\n\n\n\n<br>\n\n\n\n\n\n<p class=\"smalltext\"><a href=\"http:\/\/www.adobe.com\/products\/acrobat\/readstep2.html\"><img loading=\"lazy\" decoding=\"async\" alt=\"Download Adobe Acrobat Reader\" height=\"21\" src=\"\/wp-content\/uploads\/dhss\/img\/getacro60.gif\" width=\"60\"><\/a> Please note: Some of the files available on this page are in Adobe PDF format which requires Adobe Acrobat Reader.
