ENHANCED INFLUENZA SURVEILLANCE IN THE UNITED STATES\n\n\n<!--[END]-->\n\n<\/h2>\n\n\n\n<!--[type=HTMLEDIT name=\"Content\"]-->\n\n\n\n<p>\n\n\n  The Centers for Disease Control and Prevention has alerted state health agencies of increasing concerns regarding human transmission of\n\n\n  avian influenza. The Delaware Division of Public Health (DPH) is providing this Health Advisory to assure that Delaware health care\n\n\n  providers are aware of the emerging threat.\n\n\n<\/p>\n\n\n<p>\n\n\n  While there is no evidence of avian influenza in humans or birds in Delaware at this time, DPH is increasing surveillance efforts:\n\n\n<\/p>\n\n\n<ul>\n\n\n  <li>Active, daily syndromic surveillance &#8211; In response to Severe Acute Respiratory Syndrome (SARS) and general concerns about\n\n\n  bioterrorism, Delaware hospitals are already reporting daily the number of patients treated for specific respiratory symptoms, consistent\n\n\n  with symptoms of human avian influenza. Significant increases in the number of hospital patients with such symptoms are investigated by\n\n\n  DPH epidemiologists.\n\n\n  <\/li>\n\n\n  <li>Sentinel influenza surveillance &#8211; DPH is seeking to expand its network of participants in the sentinel influenza surveillance\n\n\n  program. Currently, four physician-practices submit nasopharyngeal swabs from patients with influenza-like symptoms to the DPH laboratory\n\n\n  for viral testing. Influenza, when identified, is typed by the DPH laboratory. The DPH laboratory analysis is provided at no cost to the\n\n\n  physician or patient. Physician-practices interested in participating in this program, especially from New Castle and Sussex Counties,\n\n\n  should contact Dr. Leroy Hathcock, State Epidemiologist at (302) 744-4703 or at leroy.hathcock@delaware.gov for further information.\n\n\n  <\/li>\n\n\n<\/ul>\n\n\n<p>\n\n\n  In addition, the Delaware Department of Agriculture conducts surveillance activities for the presence of avian influenza in chickens and\n\n\n  will alert DPH of any findings.\n\n\n<\/p>\n\n\n<p>\n\n\n  Questions about this Health Advisory can be directed to DPH at 1-888-295-5156.\n\n\n<\/p>\n\n\n<h2 class=\"text-center\">\n\n\n  This is an official<br>\n\n\n  CDC Health Update\n\n\n<\/h2>\n\n\n<p>\n\n\n  Distributed via Health Alert Network<br>\n\n\n  April 28, 2003, 17:43 EDT ( 5:43 PM EDT)<br>\n\n\n  CDCHAN-00138-03-04-28-UPD-N\n\n\n<\/p>\n\n\n<h3 class=\"text-center\">\n\n\n  Human Cases of Avian Influenza A (H7N7) Infection, <em>The Netherlands<\/em><br>\n\n\n  Enhanced Influenza Surveillance in the United States\n\n\n<\/h3>\n\n\n<p>\n\n\n  The Netherlands has been reporting outbreaks of highly pathogenic avian influenza A (H7N7) among poultry on several farms since the end\n\n\n  of February 2003. More recently cases of H7N7 infection have been reported among pigs and humans in the Netherlands, and among birds in\n\n\n  Belgium.\n\n\n<\/p>\n\n\n<p>\n\n\n  As of April 25, 2003, the National Influenza Center in the Netherlands reported that 83 confirmed cases of human H7N7 influenza virus\n\n\n  infection had occurred among poultry workers and their families since late February 2003. Most of the infected persons had clinical signs\n\n\n  of conjunctivitis, and some had mild influenza-like illness. However, one individual, a 57-year-old veterinarian who visited one of the\n\n\n  affected farms in early April, died on April 17 of acute respiratory distress syndrome (ARDS) and related complications from H7N7\n\n\n  infection. Dutch authorities also have reported evidence of possible transmission of H7N7 influenza from 2 poultry workers to 3 family\n\n\n  members who developed mild illness. Additional information is available on the Centers for Disease Control and Prevention website at:\n\n\n  <a href=\"http:\/\/www.cdc.gov\/ncidod\/diseases\/flu\/H7N7facs.htm\">http:\/\/www.cdc.gov\/ncidod\/diseases\/flu\/H7N7facs.htm<\/a>\n\n\n<\/p>\n\n\n<p>\n\n\n  This is the second reported occurrence of human infection with avian influenza viruses since early February 2003, when 2 human cases of\n\n\n  influenza A (H5N1) infection were confirmed in a single family of Hong Kong residents who had recently traveled to Fujian Province on\n\n\n  mainland China. Both patients were hospitalized and one died. A third family member died while in China, but testing was not done to\n\n\n  determine the cause of her death. There is currently insufficient information to determine whether this family was infected from a common\n\n\n  source or whether illness spread within the family from person to person.\n\n\n<\/p>\n\n\n<p>\n\n\n  Both influenza A (H5N1) and influenza A (H7N7) are influenza viruses that have not circulated widely among people in the past; therefore,\n\n\n  the general population has little or no immunity to the viruses. When a novel influenza virus infects a person, the situation is\n\n\n  monitored closely because of the potential for an influenza pandemic in the event that the virus causes illness and is able to spread\n\n\n  efficiently from person to person. Three influenza pandemics occurred in the 20<sup>th<\/sup> century, the most devastating of which was\n\n\n  the pandemic of 1918-19, during which more than 550,000 people in the United States died of influenza-related complications.\n\n\n<\/p>\n\n\n<p>\n\n\n  In response to the 2 human cases of H5N1 in February 2003, CDC issued recommendations for enhanced influenza surveillance for state\n\n\n  health departments (<a href=\"http:\/\/www.cdc.gov\/ncidod\/diseases\/flu\/hanH5N1.htm\">http:\/\/www.cdc.gov\/ncidod\/diseases\/flu\/hanH5N1.htm<\/a>).\n\n\n  The purpose of these recommendations is to enhance the capacity to rapidly identify an importation of a novel strain of influenza A virus\n\n\n  into the United States, while minimizing the disruption of existing public health response capacity. The surveillance enhancements are\n\n\n  structured to occur in a stepwise fashion based on the evolution of the influenza activity associated with the novel virus.\n\n\n<\/p>\n\n\n<p>\n\n\n  The current situation regarding human cases of influenza H7N7 in Europe further points to the importance of conducting enhanced influenza\n\n\n  surveillance as was outlined in the CDC recommendations issued earlier this year. Based on current knowledge of the H5N1 and H7N7\n\n\n  situations, CDC recommends:\n\n\n<\/p>\n\n\n<ul>\n\n\n  <li>All US World Health Organization\/National Respiratory and Enteric Virus Surveillance System collaborating laboratories should subtype\n\n\n  <strong><em>all<\/em><\/strong> influenza A viruses identified in clinical specimens, if possible, and should report any unsubtypable\n\n\n  influenza A virus to the CDC Influenza Branch immediately.\n\n\n  <\/li>\n\n\n  <li>States at less than 75% of their sentinel provider goal of 1 regularly reporting site per 250,000 population (or a minimum of 10\n\n\n  sites in smaller, less populous states) should consider recruiting and enrolling additional providers to allow for tracking the\n\n\n  occurrence and intensity of any potential pandemic influenza A activity in their state.\n\n\n  <\/li>\n\n\n  <li>States should continue both laboratory and sentinel provider surveillance activities year round, uninterrupted.\n\n\n  <\/li>\n\n\n<\/ul>\n\n\n<p>\n\n\n  There is considerable overlap between the clinical presentation and travel history of persons who may have severe acute respiratory\n\n\n  syndrome (SARS) and those who should be evaluated for infection with influenza A (H5N1). Influenza A infection should be considered in\n\n\n  the differential diagnosis when evaluating a SARS patient. Priority should be given to subtyping influenza A viruses isolated from\n\n\n  potential SARS cases. If any such viruses cannot be subtyped, the CDC Influenza Branch should be notified immediately.\n\n\n<\/p>\n\n\n<p>\n\n\n  By early April, influenza A (H5N1) reagents were distributed to all US WHO Collaborating Laboratories, including the state public health\n\n\n  laboratories.
