This Update provides information about:\n\n\n<\/p>\n\n\n<ul>\n\n\n  <li>specimen collection and analysis in Delaware\n\n\n  <\/li>\n\n\n  <li>a revised case definition from the Centers for Disease Prevention and Control (CDC)\n\n\n  <\/li>\n\n\n<\/ul>\n\n\n<p>\n\n\n  Questions about this update can be directed to DPH at 1-888-295-5156.\n\n\n<\/p>\n\n\n<h3>\n\n\n  SPECIMEN COLLECTION AND ANALYSIS\n\n\n<\/h3>\n\n\n<p>\n\n\n  Clinical specimens for patients meeting the case definition below should be submitted to the Delaware DPH laboratory. DPH is preparing to\n\n\n  conduct both EIA for detection of antibody in serum and PCR for molecular detection of virus in clinical specimens. These tests should be\n\n\n  available by mid-June. The DPH Laboratory can also coordinate submission of specimens for laboratory analysis by CDC. Contact the DPH\n\n\n  Laboratory for further information at (302) 653-2870 about specimen collection and consent forms, and related procedures.\n\n\n<\/p>\n\n\n<h2 class=\"text-center\">\n\n\n  This is an official<br>\n\n\n  CDC Health Update\n\n\n<\/h2>\n\n\n<h3 class=\"text-center\">\n\n\n  Revisions to the Interim U.S. Case Definition for<br>\n\n\n  Severe Acute Respiratory Syndromes<br>\n\n\n  June 4, 2003\n\n\n<\/h3>\n\n\n<p>\n\n\n  The previous CDC SARS case definition (published May 23, 2003) has been updated. The last date for illness onset for persons who meet the\n\n\n  clinical criteria for SARS and who report travel to Singapore have been revised. laboratory criteria confirmed, negative, or undetermined.\n\n\n  <\/li>\n\n\n  <li>Suspect case: meets the clinical criteria for moderate respiratory illness of unknown etiology, and epidemiologic criteria for\n\n\n  exposure; laboratory criteria confirmed, negative, or undetermined.\n\n\n  <\/li>\n\n\n<\/ul>\n\n\n<h5>\n\n\n  Exclusion Criteria\n\n\n<\/h5>\n\n\n<p>\n\n\n  A case may be excluded as a suspect or probable SARS case if:\n\n\n<\/p>\n\n\n<ul>\n\n\n  <li>An alternative diagnosis can fully explain the illness***\n\n\n  <\/li>\n\n\n  <li>The case was reported on the basis of contact with an index case that was subsequently excluded as a case of SARS (e.g., another\n\n\n  etiology fully explains the illness) provided other possible epidemiologic exposure criteria are not present\n\n\n  <\/li>\n\n\n<\/ul>\n\n\n<p>\n\n\n  * A measured documented temperature of >100.4\u00ba F (>38\u00ba C) is preferred. However, clinical judgment should be used when\n\n\n  evaluating patients for whom a measured temperature of >100.4\u00ba F (>38\u00ba C) has not been documented. Factors that might be\n\n\n  considered include patient self-report of fever, use of antipyretics, presence of immunocompromising conditions or therapies, lack of\n\n\n  access to health care, or inability to obtain a measured temperature. Reporting authorities should consider these factors when\n\n\n  classifying patients who do not strictly meet the clinical criteria for this case definition.\n\n\n<\/p>\n\n\n<p>\n\n\n  \u00a7 Close contact is defined as having cared for or lived with a person known to have SARS or having a high likelihood of direct\n\n\n  contact with respiratory secretions and\/or body fluids of a patient known to have SARS. Examples of close contact include kissing or\n\n\n  embracing, sharing eating or drinking utensils, close conversation (<3 feet), physical examination, and any other direct physical\n\n\n  contact between persons. Close contact does not include activities such as walking by a person or sitting across a waiting room or office\n\n\n  for a brief period of time.\n\n\n<\/p>\n\n\n<p>\n\n\n  \u2021 The WHO has specified that the surveillance period for China should begin on November 1; the first recognized cases in Hong\n\n\n  Kong, Singapore and Hanoi (Vietnam) had onset in February 2003. The dates for Toronto and Taiwan are linked to CDC\u2019s issuance of\n\n\n  travel recommendations.\n\n\n<\/p>\n\n\n<p>\n\n\n  \u2020 The last date for illness onset is 10 days (i.e., one incubation period) after removal of a CDC travel alert. The case\n\n\n  patient\u2019s travel should have occurred on or before the last date the travel alert was in place.\n\n\n<\/p>\n\n\n<p>\n\n\n  \u00b6 Assays for the laboratory diagnosis of SARS-CoV infection include enzyme-linked immunosorbent assay, indirect fluorescent-antibody\n\n\n  assay, and reverse transcription polymerase chain reaction (RT-PCR) assays of appropriately collected clinical specimens (Source: CDC.\n\n\n  Guidelines for collection of specimens from potential cases of SARS. Absence of SARS-CoV antibody from serum obtained <21 days after\n\n\n  illness onset, a negative PCR test, or a negative viral culture does not exclude coronavirus infection and is not considered a definitive\n\n\n  laboratory result. In these instances, a convalescent serum specimen obtained >21 days after illness is needed to determine infection\n\n\n  with SARS-CoV.
