Reporting enables appropriate public\r\nhealth follow-up for your patients, helps identify outbreaks, and provides a better understanding of disease trends in Delaware.\r\n<h3>How to Report<\/h3>\r\n<ul>\r\n \t<li>Complete a Hepatitis C Case Report Form ( <a href=\"\/wp-content\/uploads\/sites\/10\/dph\/pdf\/hepccasereport.pdf\">PDF<\/a> ).
