With a $157 million federal award for the first year alone, this program has the potential to revolutionize medical care in every community. \r\n      <\/p>\r\n<p>Our objective \u2014 to improve health outcomes for all Delawareans, regardless of income or zip code. <\/p> \r\n\r\n      <figure class=\"rhtp-quote\" aria-label=\"Quote\">\r\n        <blockquote>\r\n          <em>\u201cA generational plan focused on expanding access to care and supporting rural communities from the ground up.\u201d<\/em>\r\n        <\/blockquote>\r\n        <figcaption><strong>Governor Matt  <p>All Delawareans deserve access to quality health care, no matter where they live.<\/p>\r\n      <p>Delaware\u2019s Rural Health Transformation Program includes 15 projects, programs and initiatives that will:<\/p>\r\n    <\/div>\r\n  <\/section>\r\n\r\n  <section class=\"rhtp-section\" aria-labelledby=\"rhtp-projects\">\r\n    <div class=\"rhtp-container\">\r\n      <h2 class=\"rhtp-h2\" id=\"rhtp-projects\">Projects, Programs, and Initiatives<\/h2>\r\n\r\n      <div class=\"rhtp-accordion\" role=\"region\" aria-label=\"Rural Health projects and class=\"rhtp-card\">\r\n                <h3 class=\"rhtp-h3\">School-Based Health Centers <\/h3>\r\n                <p>Funds will be competitively awarded to support operation of new school-based health centers. <\/p>\r\n<br>\r\n<p><em><strong>Year 1 Budget: $195,000.00<\/strong><\/em><\/p>\r\n              <\/li>\r\n\r\n              <li class=\"rhtp-card\">\r\n                <h3 class=\"rhtp-h3\">Libraries<\/h3>\r\n                <p>Funds will be competitively awarded to rural libraries for support services for patients engaging in telehealth. <\/p>\r\n<br>\r\n<p><em><strong>Year 1 Budget: $1,835,000.00<\/strong><\/em><\/p>\r\n              <\/li>\r\n\r\n            <\/ul>\r\n          <\/div>\r\n        <\/details>\r\n\r\n        <!-- 2 class=\"rhtp-card\">\r\n                <h3 class=\"rhtp-h3\">Hope Centers<\/h3>\r\n                <p>Funds will support establishment and operation of two new Hope Centers to address health needs of rural homeless population. <\/p>\r\n<br>\r\n<p><em><strong>Year 1 Budget: $26,370,000.00<\/strong><\/em><\/p>\r\n              <\/li>\r\n\r\n              <li class=\"rhtp-card\">\r\n                <h3 class=\"rhtp-h3\">Food Is Medicine infrastructure<\/h3>\r\n                <p>Funds will be competitively awarded to a vendor to support sustainable Food Is Medicine initiatives. <\/p>\r\n<br>\r\n<p><em><strong>Year 1 Budget: $1,648,000.00<\/strong><\/em><\/p>\r\n              <\/li>\r\n\r\n              <li class=\"rhtp-card\">\r\n                <h3 class=\"rhtp-h3\">Continuous glucose monitoring<\/h3>\r\n                <p>Funds will be competitively awarded to a vendor to pilot a continuous glucose monitoring program. <\/p>\r\n<br>\r\n<p><em><strong>Year 1 Budget: $950,000.00<\/strong><\/em><\/p>\r\n              <\/li>\r\n\r\n              <li class=\"rhtp-card\">\r\n                <h3 class=\"rhtp-h3\">Telehealth and remote patient monitoring<\/h3>\r\n                <p>Funds will be competitively awarded to vendors deploying innovative telehealth and remote patient monitoring costs for a medical school branch campus. <\/p>\r\n<br>\r\n<p><em><strong>Year 1 Budget: $42,500,000.00<\/strong><\/em><\/p>\r\n              <\/li>\r\n\r\n              <li class=\"rhtp-card\">\r\n                <h3 class=\"rhtp-h3\">Support medical students<\/h3>\r\n                <p>Tuition awards will go to medical students who commit to 5 years practice in rural Delaware. non-physician clinicians<\/h3>\r\n                <p>Tuition awards for non-physician health care workers who commit to serve in rural Delaware. <\/p>\r\n<br>\r\n<p><em><strong>Year 1 Budget: FQHCs to move towards value-based care, including preparing to take on upside and downside risk and be held accountable for total cost of care and population health outcomes.\r\n
