Our program supports community clinical linkages, health systems interventions, environmental approaches, and epidemiology and surveillance.<\/p>\r\n\r\n\r\n\r\n\r\n\r\n\r\n\r\n<p>The program is funded by a cooperative agreement with the Centers for Disease Control and Prevention (CDC), with additional support from the Delaware Health Fund. The program collects and publicizes current, accurate information about diabetes and heart disease, develops approaches for reducing the impact of the diseases, promotes healthy lifestyle habits for prevention and control, and coordinates efforts of public and private health organizations.<\/p>\r\n\r\n\r\n\r\n\r\n\r\n\r\n\r\n<h2>About Delaware\u2019s Diabetes and Heart Disease
