and vaccinations<\/strong>;<\/li>\n\n\n\n\n\n <li>Provide <strong>speakers, programs, products, and educational materials<\/strong> for people with diabetes, pre-diabetes, heart disease,\u00a0or who are at risk of developing these diseases;<\/li>\n\n\n\n\n\n <li>Provide <strong>nutritional education programs<\/strong> for people with, or at risk for,\u00a0chronic health conditions, and for care givers and health professionals;<\/li>\n\n\n\n\n\n <li>Promote the use of <strong>Electronic\u00a0Health Records <\/strong>for early identification and to\u00a0improve the quality of health for patients with uncontrolled diabetes and hypertension;<\/li>\n\n\n\n\n\n <li>Promote early <strong>detection and diagnosis of diabetes and hypertension\u00a0in clinical settings<\/strong>;<\/li>\n\n\n\n\n\n <li>Conduct <strong>public awareness campaigns focusing on prevention or control<\/strong> of diabetes and hypertension,\u00a0and\u00a0their complications;<\/li>\n\n\n\n\n\n <li>Support <strong>community screenings <\/strong>for diabetes, pre-diabetes and\/or hypertension; and<\/li>\n\n\n\n\n\n <li><strong>Collect and disseminate data<\/strong> on the burden of diabetes and heart disease in Delaware.<\/li>\n\n\n\n\n\n<\/ul>\n\n\n\n\n\n\n\n\n\n\n\n<h2>Mobile Diabetes and Hypertension Wellness Initiative<\/h2>\n\n\n\n\n\n\n\n\n\n\n\n<p>The purpose of this initiative is to <strong>identify individuals at high risk for hypertension (high blood pressure) and\/or diabetes within the worksite setting<\/strong>. The Initiative works to educate them on the importance of disease management and to increase their utilization of standard preventive exams and\/or tests. We target companies that employ between 100 \u2013 500 employees and currently have\u00a0few or no formal health promotion initiatives in place.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n<p>The initiative is designed to include facilities in all three counties in the state. Any participating employee with \u201cabnormal\u201d results from the screening is referred to their healthcare provider according to the protocol agreed on with company representatives. Many of the onsite activity stations include interactive displays and or suggested activities, in addition to handouts and other resources.
