have included the administering of a child&#8217;s immunizations in their medical home (their regular source of primary care).<\/p>\n\n\n\n\n\n\n\n\n <\/li>\n\n\n\n\n\n\n\n\n<\/ul>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p><strong>How do I become a VFC provider?<\/strong><\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<ul>\n\n\n\n\n\n\n\n\n <li>\n\n\n\n\n\n\n\n\n <p>First, contact the Immunization Program\u2019s VFC Coordinator at (302) 744-1060 or email at immunizedph@state.de.us for a Provider Enrollment Package to be mailed to you.<\/p>\n\n\n\n\n\n\n\n\n <\/li>\n\n\n\n\n\n\n\n\n <li>\n\n\n\n\n\n\n\n\n <p>Complete the States Provider Enrollment form and return them as soon as possible.<\/p>\n\n\n\n\n\n\n\n\n <\/li>\n\n\n\n\n\n\n\n\n <li>\n\n\n\n\n\n\n\n\n <p>Prepare your office and staff for a site visit to go over the administrative requirements of the program and to ensure proper storage and handling of vaccines when you receive them.<\/p>\n\n\n\n\n\n\n\n\n <\/li>\n\n\n\n\n\n\n\n\n <li>\n\n\n\n\n\n\n\n\n <p>Let your patients know you are now a VFC Provider.<\/p>\n\n\n\n\n\n\n\n\n <\/li>\n\n\n\n\n\n\n\n\n<\/ul>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p><strong>Is there any financial incentive?
