There are 18 identifiers specified by HIPAA\n\n\n\n\n\nthat \n\n\n\n\n\nare considered to be PHI and therefore require an individual\u2019s authorization to access their information,\n\n\n\n\n\n\n\n\n\n\n\ngranted by the provider of the PHI, or obtain an approval of a waiver of authorization.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n<p>The 18 HIPAA Identifiers are:<\/p>\n\n\n\n\n\n<ol>\n\n\n\n\n\n<li>Names;<\/li>\n\n\n\n\n\n<li>All geographical subdivisions smaller than a State, including street address, city, county,\n\n\n\n\n\nprecinct, zip code, and their equivalent geocodes, except for the initial three digits of a zip code,\n\n\n\n\n\nif according to the current publicly available data from the Bureau of the Census: (1) The geographic\n\n\n\n\n\nunit formed by combining all zip codes with the same three initial digits contains more than 20,000 people;\n\n\n\n\n\nand (2) The initial three digits of a zip code for all such geographic units containing 20,000 or fewer\n\n\n\n\n\npeople is changed to 000.
