By answering all of the subsequent questions,\n\n\n\n\nyou will be directed to the data application form appropriate to your needs. Each form contains instructions\n\n\n\n\non how to complete and submit the application. A general description of the types of data is provided\n\n\n\n\nbelow. Please refer back to each category of data type under the Data Guidelines section on the\n\n\n\n\nhome page for more specific guidance.<\/p>\n\n\n\n\n\n\n\n\n\n<p><a href=\"drppublic.html\"><strong>Public Use Data<\/strong><\/a> is aggregated or record level data that has\n\n\n\n\nbeen stripped of the 18 identifiers specified by HIPPA and therefore is not considered Protected Health Information (PHI). \n\n\n\n\nVisit <a href=\"drpphi.html\">the PHI page<\/a> to view a list of these identifiers. If you are seeking public use data, \n\n\n\n\nanswer yes to the below question and enter your request in the box provided below it. Depending on the type of\n\n\n\n\npublic data desired, the request will be directed to staff in the relevant DPH section to fulfill.<\/p>\n\n\n\n\n\n\n\n\n\n<p>If published reports or aggregate data tables will not provide the information needed, record level\n\n\n\n\ndata files may be requested. The application process to request record level data varies based on who\n\n\n\n\nthe requester is, the type of data (protected, limited access, or public use), and contact with human\n\n\n\n\nsubjects (see <a href=\"\/wp-content\/uploads\/sites\/10\/dph\/pdf\/drphsrbprocguidelines.pdf\">Human Subjects\n\n\n\n\nReview Board Procedures\/Guidance<\/a>).<\/p>\n\n\n\n\n\n\n\n\n\n<p><a href=\"drplimited.html\"><strong>Limited data<\/strong><\/a> do not contain personal identifiers, but\n\n\n\n\nthey do contain individual specific data such as city, zip code, census tract, elements of dates relegated to a person, \n\n\n\n\nand other unique characteristics.<\/p>\n\n\n\n\n\n\n\n\n\n<p><a href=\"drpprotected.html\"><strong>Protected data<\/strong><\/a> includes confidential, personal identifiers\n\n\n\n\nsuch as name, Social Security Number, and full address. Requests for Protected data will require additional time for review and approval.\n\n\n\n\nAny requested research project involving a DHSS\/DPH client must also be approved by the HSRB.<\/p>\n\n\n\n\n\n\n\n\n\n<p>Some data requests, once submitted, may need to be channeled through the <a href=\"drpfoia.html\">Freedom\n\n\n\n\nof Information Act (FOIA)<\/a> process.
