There are three color-coded collection kits available from the DPHL warehouse. Store collection kits at Room temperature (15<\/span><span style=\"font-family:\"Cambria Math\",serif;font-size:12.0pt;color:#333333;\">\u2103<\/span><span style=\"font-family:\"Open Sans\",sans-serif;\">\u00a0\u2013 30<\/span><span style=\"font-family:\"Cambria Math\",serif;\">\u2103<\/span><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">).<\/span><\/li>\n\n\n\n            <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Urine Kit \u2013 for urine specimens only (YELLOW)<br>\u00a0Multitest Swab Kit \u2013 for endocervical, male urethral, patient-collected vaginal, oral and rectal swabs (ORANGE)<\/span><\/li>\n\n\n\n            <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Specimens must be labeled with at least two unique identifiers (i.e., patient name, date of birth, collection date, and\/or clinic or school name) using the &#8220;B&#8221; barcodes printed from the Laboratory Information Management System (LIMS). Specimens must be placed in a biohazard bag for transport (only one specimen per bag).<\/span><\/li>\n\n\n\n            <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Complete the\u00a0<\/span><span style=\"color:black;\"><a href=\"\/wp-content\/uploads\/sites\/10\/dph\/pdf\/testreq.pdf\"><span style=\"font-family:\"Open Sans\",sans-serif;\"><b>Test Requisition Form<\/b><\/span><\/a><\/span><span style=\"font-size:12.0pt;color:#333333;\">\u00a0including patient history information and insurance information (include ICD-9 code and Medicaid number) or order directly in LIMS as you would normally submit orders for laboratory tests.<\/span><\/li>\n\n\n\n            <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Specimens and forms should be sent to the laboratory, preferably the same day of collection. (If using a lab drop site, be sure that specimens will be picked up within the acceptable time frame (some sites do not have daily pickups).<\/span><\/li>\n\n\n\n            <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Male and Female Urine Directions \u2013 Use Aptima Urine Collection Kit<\/span>\n\n\n\n                <ol style=\"list-style-type: lower-alpha;\">\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">The patient should not have urinated for at least one hour prior to sampling.<\/span><\/li>\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;\">Direct patient to provide a first-catch urine (approximately 20 to 30 mL of the initial urine stream) into a urine collection cup. Collection of larger volumes of urine may reduce test sensitivity. Female patients should not cleanse the labial area prior to providing the specimen.<\/span><\/li>\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Remove the cap and transfer 2 mL of urine into the urine specimen transport tube using the disposable pipette provided. The correct volume of urine has been added when the fluid level is between the black lines on the urine specimen transport tube label.<\/span><\/li>\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;\">Re-cap the urine specimen transport tube tightly. This is now known as the processed urine specimen.<\/span><\/li>\n\n\n\n                <\/ol>\n\n\n\n            <\/li>\n\n\n\n            <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Self-Penile Swab Collection Directions \u2013 Use Multitest Swab Collection Kit<\/span>\n\n\n\n                <ol style=\"list-style-type: lower-alpha;\">\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Partially peel open the swab package. Remove the swab. Do not touch the soft tip or lay the swab down.<\/span><\/li>\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;\">Hold swab in your hand, placing your thumb and forefinger in the middle of the swab shaft covering the score line (black line). Do not hold swab shaft below the score line (black line).<\/span><\/li>\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Uncircumcised men will have to roll the foreskin down before starting collection. Hold the penis with your free hand (hand with no swab). Using your other hand (with the swab), roll the swab just at the tip or outside the opening to the penis. It is not necessary to put the swab deep inside the opening of the penis.<\/span><\/li>\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;\">While holding the swab in the same hand, unscrew the cap from the swab specimen transport tube, making sure not to spill any liquid. Immediately place the specimen collection swab into the transport tube.<\/span><\/li>\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;\">Carefully break the swab shaft at the score line; use care to avoid splashing of contents.<\/span><\/li>\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Re-cap the swap specimen transport tube tightly.<\/span><\/li>\n\n\n\n                    <li><span style=\"font-family:\"Open Sans\",sans-serif;font-size:12.0pt;color:#333333;\">Wash hands with soap and water.<\/span><\/li>\n\n\n\n                <\/ol>\n\n\n\n            <\/li>\n\n\n\n        <\/ol>\n\n\n\n    \n\n\n\n    \n\n\n\n        <p style=\"margin-top:0in;margin-right:0in;margin-bottom:8.0pt;margin-left:60.75pt;font-size:11.0pt;font-family:\"Calibri\",sans-serif;line-height:25.2pt;background:white;\"><span style=\"font-size:16px;font-family:\"Open Sans\",sans-serif;color:#333333;\"><br>\u00a0<\/span><\/p>\n\n\n\n    \n\n\n\n\n\n\n\n<h2>\n\n\n\n  SPECIMEN STABILITY AND TRANSPORT\n\n\n\n<\/h2>\n\n\n\n<p>\n\n\n\n    <\/p><ol style=\"list-style-type: lower-alpha\">\n\n\n\n\n\n\n\n<li> Urine \u2013 After collection, transport, and store urine specimen transport tube at 2\u2103 to 30\u2103 until tested.  Specimens must be tested using the CT\/GC NAAT method within 30 days of collection.  If longer storage is needed, freeze at -20\u2103 to -70\u2103 for up to 90 days after collection for CT\/GC and up to 24 months for Trichomonas vaginalis.\n\n\n\n      <\/li>\n\n\n\n<li> Swab (any source) \u2013 After collection, transport, and store swab in transport tube at 2\u2103 to 30\u2103 until tested.  Specimens must be tested using the Assay for CT\/GC and\/or TV within 60 days of collection.  If longer storage is needed, freeze at -20\u2103 to -70\u2103 for up to 90 days after collection for CT\/GC and up to 24 months for Trichomonas vaginalis.\n\n\n\n<\/li>\n\n\n\n<\/ol><p>\n\n\n\n<br>\n\n\n\n\n\n\n\n<\/p><h2>RESULTS<\/h2><p>\n\n\n\nResults are routinely available within 72 hours of receipt in laboratory. Final reports are available in LIMS as they are resulted. This test specifically targets the DNA of these microorganisms and can detect the DNA whether they are alive or dead. Retesting should only be performed after 3 weeks from the start of any treatment ordered by the providing clinician. \n\n\n\n<\/p><p>\n\n\n\n<br>\n\n\n\n\n\n\n\n<\/p><h2>REJECTION<\/h2><h3>Samples will be rejected if they are:<\/h3><ul>\n\n\n\n  <li>Unlabeled specimens or those with discrepancies between specimen and requisition forms \u2013 All specimens MUST have a unique patient\n\n\n\n  identifier &#8220;B&#8221; barcode from LIMS.\n\n\n\n  <\/li>\n\n\n\n  <li>Insufficient in Quantity \u2013 No specimen received, no specimen in container, or insufficient specimen to perform testing. Specimens\n\n\n\n  received with no orders or requisition forms.\n\n\n\n  <\/li>\n\n\n\n  <li>Improperly Preserved \u2013 Specimens must be refrigerated or stored at room temp for 30 days as defined by the laboratory. Tubes have\n\n\n\n  collection media in the container that must be maintained in the tube with the swab. Expired transport containers, specimens received\n\n\n\n  after prolonged delay or inappropriate transport conditions.\n\n\n\n  <\/li>\n\n\n\n  <li>Damaged \u2013 Specimen leaked or broken in transit, dry specimens, or in fixative or other additive.\n\n\n\n  <\/li>\n\n\n\n  <li>Wrong swab \u2013 the collection kit only has a pink swab that can be used for testing.
