Disabilities is working with numerous partner agencies to develop and\u00a0implement Delaware&#8217;s Aging and Disability Resource Center (ADRC). Eight\u00a0entities have been designated as lead partners in the project.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>This page provides information about the <a class=\"anchors_link\" href=\"#agencies\">Lead Partner Agencies<\/a>, including the roles that they will play in supporting the ADRC. It also includes information about the <a class=\"anchors_link\" href=\"#meetings\">Lead Partner Meetings<\/a> to be held throughout the ADRC planning and develpment process.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h2>Lead Partner Agencies<\/h2>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h3><a href=\"\/dmma\/\">Division of Medicaid and Medical Assistance<\/a>\u00a0<\/h3>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>The Division\u00a0of Medicaid and Medical Assistance (DMMA), one of twelve divisions within the Department\u00a0of Health and Social Services, provides medical services to vulnerable populations in Delaware.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>DMMA\u00a0will carry out the following activities to support the ADRC: 1) provide training for ADRC staff and partners on technical issues surrounding various Medicaid programs to enable the ADRC to better assist consumers and caregivers with Medicaid-related questions and\/or applications; 2) provide the ADRC and partners with updates about important changes in Medicaid programs; 3) coordinate with the ADRC to expedite Medicaid applications through the ADRC.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h3><a href=\"https:\/\/insurance.delaware.gov\/divisions\/dmab\/\">Department of Insurance, ELDERinfo<\/a>\u00a0<\/h3>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>As the State Health Insurance Counseling and Assistance Program (SHIP) for Delaware, ELDERinfo provides counseling and assistance on questions and problems related to Medicare, Medicaid, Medigap, long-term care insurance and other types of health insurance.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>ELDERinfo will carry out the following activities to support the ADRC: 1) provide training for ADRC staff on various aspects of the ELDERinfo program; 2) educate ELDERinfo volunteers about the ADRC; 3) distribute ADRC outreach materials to consumers; 4) put a link to the ADRC web site on the ELDERinfo web site; 5) make referrals to and accept referrals from the ADRC.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h3><a href=\"http:\/\/www.fcilde.org\/\">Freedom Center for Independent Living, Inc.<\/a><\/h3>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>Freedom Center for Independent Living is one of two Centers for Independent Living (CILs) in Delaware. CIL&#8217;s provide a range of services to persons with disabilities, including\u00a0advocacy, peer support, information and referral, and independent living skills training.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>The\u00a0Freedom Center for Independent Living\u00a0will carry out the following activities to support the ADRC: 1) provide assistance in planning and developing ADRC materials; 2) engage in cross-training with ADRC staff ; 3) share information regarding services, providers and resources to assist in maintaining and updating the ADRC database; 4) provide information regarding the unmet needs of older persons and adults with physical disabilities; 5) collaborate with DSAAPD on community events and outreach related to the ADRC; 6) make referrals to and accept referrals from the ADRC.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h3><a href=\"http:\/\/www.iri-de.org\/\">Independent Resources, Inc.<\/a><\/h3>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>Independent\u00a0 Resources, Inc. is one of two Centers for Independent Living (CILs) in Delaware. As noted above, CIL&#8217;s provide a range of services to persons with disabilities, including\u00a0advocacy, peer support, information and referral, and independent living skills training.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>Independent Resources, Inc.\u00a0will carry out the following activities to support the ADRC: 1) provide assistance in planning and developing ADRC materials; 2) engage in cross-training with ADRC staff ; 3) share information regarding services, providers and resources to assist in maintaining and updating the ADRC database; 4) provide information regarding the unmet needs of older persons and adults with physical disabilities; 5) collaborate with DSAAPD on community events and outreach related to the ADRC; 6)make referrals to and accept referrals from the ADRC.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h3><a href=\"http:\/\/www.delawareagingnetwork.org\/\">Delaware Aging Network<\/a><\/h3>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>The Delaware Aging Netwrk (DAN) is a consortium of over 50 agencies dedicated to improving the quality of services for older Delawareans. In addition to operating and coordinating services, DAN also advocates for policy changes that benefit the aging population.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>DAN will will carry out the following activities to support the ADRC: 1) provide assistance in planning and developing ADRC materials; 2) engage in cross-training with ADRC staff ; 3) share information regarding services, providers and resources to assist in maintaining and updating the ADRC database; 4) provide information regarding the unmet needs of older persons and adults with physical disabilities; 5) collaborate with DSAAPD on community events and outreach related to the ADRC; 6) make referrals to and accept referrals from the ADRC.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h3><a href=\"https:\/\/christianacare.org\/\">Christiana Care Health System<\/a><\/h3>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>The Christiana Care Health System is one of the largest health care providers in the mid-Atlantic region, offering a range of services including more than 1,100 licensed beds in two hospitals.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>The Christiana Care Health System will carry out the following activities to support the ADRC: 1) provide information regarding the unmet post-acute care needs of older persons and adults with physical disabilities in Delaware; 2) provide input into the development of ADRC materials to be used in the hospital discharge process; 3) engage in cross-training with ADRC staff;\u00a04) distribute information provided by the ADRC to patients and caregivers; 5)\u00a0Coordinate with ADRC partner agencies selected by DSAAPD to support discharge planning efforts.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h3><a href=\"https:\/\/scpd.delaware.gov\/\">State Council for Persons with Disabilities<\/a><\/h3>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>The State Council for Persons with Disabilities (SCPD) is charged with the responsibility of proposing and promoting laws, regulations, programs and policies which improve the lives of individuals with disabilities.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>SCPD will carry out the following activities to support the ADRC: 1) review draft materials; 2)\u00a0provide a forum for program planning; 3)\u00a0assist in outreach activities; 4)\u00a0participate in training events; 5) carry out other functions needed in support of the project.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<h3><a href=\"gac.html\">Advisory Council on Services for Aging and Adults with Physical Disabilities<\/a><\/h3>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>The Advisory Council on Services for Aging and Adults with Physical Disabilities is an advisory board comprised of older persons, adults with physical disabilities, and service professionals from around the State.
