designed for use by service professionals, including nursing home staff, hospital staff, and others to refer individuals to the <a href=\"adrc.html\">Delaware ADRC<\/a>.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>Nursing home staff: please visit the <a href=\"mds_section_q_referrals.html\">MDS 3.0 Section Q<\/a> page of this site for more information about referring nursing home residents who, through the Minimum Data Set (MDS) assessment, have expressed an interest in transitioning to a community setting.<\/p>\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n<p>Please note that referrals to the ADRC for the <a href=\"aps.html\">Adult Protective Services (APS)<\/a> program should be made <a href=\"contact.html\">by phone <\/a>rather than through the Professional Referral Form. For emergencies, please dial 911.
