legal custody or guardianship to approve medical care for that child.\u00a0<\/p>\n\n\n\n\n\n\n\n\n\n<p>The affidavit is available here for you to download. Instructions are included with the affidavit.<\/p>\n\n\n\n\n\n\n\n\n\n<h2>Download Information<\/h2>\n\n\n\n\n\n\n\n\n\n<p>The Delaware Relative Caregivers&#8217; Medical Authorization Affidavit is available here in PDF format.
