<!--[type=HTMLEDIT name=\"Content\"]-->\n\n\n\n<p>\n\n\n  &#8220;Living Will&#8221; is another name for &#8220;advance health care directive&#8221;.\u00a0Throughout this section of the site, the term\n\n\n  &#8220;advance health care directive&#8221; (or simply &#8220;advance directive&#8221;) is used, because that is the name used in the\n\n\n  Delaware law related to this subject.\n\n\n<\/p>\n\n\n<p>\n\n\n  An advance directive is established by completing an <a href=\"advance1.html\">Advance Health Care Directive Form<\/a>.\u00a0An advance\n\n\n  directive enables you to:\n\n\n<\/p>\n\n\n<ul>\n\n\n  <li>\n\n\n    <strong>Give instructions about your own health care.<\/strong><br>\n\n\n    Part I of the advance directive form lets you give specific instructions about health care decisions.\u00a0Choices are provided for you\n\n\n    to express your wishes regarding the provision, withholding, or withdrawal of treatment to keep you alive if you have a terminal\n\n\n    medical condition or if you become permanently unconscious, including the provision, withholding, or withdrawl\u00a0of artificial\n\n\n    nutrition, hydration, cardiopulmonary resuscitation, and mechanical resuscitation. Medically appropriate care necessary to ensure pain\n\n\n    relief will be provided.\u00a0Space is\u00a0also available\u00a0for you to\u00a0include any\u00a0additional health care instructions.\n\n\n  <\/li>\n\n\n  <li>\n\n\n    <strong>Name an agent to make health care decisions for you if you become incapable of making your own decisions.<\/strong><br>\n\n\n    Part II of the form allows you name another individual as an agent to make health care decisions for you if you can no longer make your\n\n\n    own decisions.\u00a0You may also name an alternate agent.\u00a0This section of the form is called a Power of Attorney for Health\n\n\n    Care.\u00a0For more details, see <a href=\"powerof.html\">What is a power of attorney for health care?<\/a>\n\n\n  <\/li>\n\n\n  <li>\n\n\n    <strong>Express an intention to donate bodily organs and\/or tissue following your death.<\/strong><br>\n\n\n    Part III of the form is optional.\u00a0It allows you, if you wish, to designate anatomical gifts to take effect upon your death.\n\n\n  <\/li>\n\n\n<\/ul>  <!--[END]-->\n\n  \n\n      <\/div>\n\n    <\/div>\n\n  <\/div><!-- \/container -->\n\n<\/div>","protected":false},"excerpt":{"rendered":"<p>Listen What are advance directives and living wills?
