By partnering with those using the service, provider staff help\n\n\n  individuals to identify goals that are meaningful to the client in his\/her recovery. ACT and ICM teams provide a comprehensive array of\n\n\n  supports that includes: clinical and rehabilitation services, employment and educational rehabilitation and supports, 24\/7 crisis\n\n\n  response, full-time psychiatrists, housing assistance, employment help, coordination of care with other community providers such as\n\n\n  medical doctors, and supports for families. The Team approach allows individuals to remain within one service system in which all\n\n\n  services are coordinated by the same staff.\n\n\n<\/p>\n\n\n<p>\n\n\n  The Eligibility and Enrollment Unit at DSAMH can answer questions regarding the system of care programs and is responsible for\n\n\n  determining if someone is eligible for services. For more information, contact the <a href=\"eeu_home.html\">Eligibility and Enrollment\n\n\n  Unit.<\/a>\n\n\n<\/p>\n\n\n<p>\n\n\n  DSAMH and its treatment providers help clients access a variety of housing programs throughout the State to assist individuals in\n\n\n  recovery from serious mental illness with short and long terms housing solutions. Access to, and eligibility for, supportive housing,\n\n\n  including Group Homes, Supervised Apartments and Residential Treatment Facilities for individuals with co-occuring mental health and\/or\n\n\n  substance abuse conditions is managed through the DSAMH Eligibility and Enrollment Unit.\n\n\n<\/p>\n\n\n<p>\n\n\n  The Eligibility and Enrollment Unit at DSAMH can answer questions regarding all client assistance programs and is responsible for\n\n\n  determining if someone is eligible for DSAMH-contracted services.
