Case Managers and nursing staff from State or contracted agencies work to determine eligibiilty and arrange for\n\n\n  those services to be provided. They also monitor these services regularly to assure that high-quality care continues.\n\n\n<\/p>\n\n\n<p>\n\n\n  There are four such Home and Community Based Waiver programs.\n\n\n<\/p>\n\n\n<h2>\n\n\n  The Elderly &#038; Disabled Home and Community Based Waiver Program:\n\n\n<\/h2>\n\n\n<p>\n\n\n  This home and community based waiver program is administered by the <a href=\"\/dsaapd\/\">Division of Services for Aging and\n\n\n  Adults with Physical Disabilities (DSAAPD)<\/a> . Services offered include all regularly covered\u00a0Medicaid services (hospital,\n\n\n  physician, lab, prescriptions) plus the following special waiver services:\n\n\n<\/p>\n\n\n<ul>\n\n\n  <li>\n\n\n    <a href=\"\/dsaapd\/perscare.html\">Personal Care<\/a>\n\n\n  <\/li>\n\n\n  <li>\n\n\n    <a href=\"\/dsaapd\/adc.html\">Adult Day\u00a0Services<\/a>\n\n\n  <\/li>\n\n\n  <li>\n\n\n    <a href=\"\/dsaapd\/ers.html\">Emergency Response System<\/a>\n\n\n  <\/li>\n\n\n  <li>\n\n\n    <a href=\"\/dsaapd\/respite.html\">Institutional or In-home Respite Care<\/a>\n\n\n  <\/li>\n\n\n<\/ul>\n\n\n<p>\n\n\n  Individuals wishing to learn more about this program should follow the links provided above.\n\n\n<\/p>\n\n\n<h2>\n\n\n  The Assisted Living Waiver Program:\n\n\n<\/h2>\n\n\n<p>\n\n\n  This home and community based waiver program is administered by the <a href=\"\/dsaapd\/\">Division of Services for Aging and\n\n\n  Adults with Physical Disabilities (DSAAPD)<\/a> . Services offered include all regularly covered Medicaid services (hospital, physician,\n\n\n  lab, prescriptions) plus the services provided in a DSAAPD approved and appropriately licensed Delaware Assisted Living Facility.\n\n\n<\/p>\n\n\n<p>\n\n\n  Individuals wishing to learn more about this program should\u00a0visit the <a href=\"\/dsaapd\/\">Assisted Living page<\/a> .\n\n\n<\/p>\n\n\n<h2>\n\n\n  The AIDS\/HIV Home and Community Based Waiver Program:\n\n\n<\/h2>\n\n\n<p>\n\n\n  This home and community based waiver program is administered by the Division of Social Services. Services offered include all regularly\n\n\n  covered Medicaid services (hospital, physician, lab,\u00a0prescriptions) plus the following special waiver services:\n\n\n<\/p>\n\n\n<ul>\n\n\n  <li>Case Management\n\n\n  <\/li>\n\n\n  <li>Personal Care services\n\n\n  <\/li>\n\n\n  <li>Respite Care\n\n\n  <\/li>\n\n\n  <li>Supplemental Nutrition\n\n\n  <\/li>\n\n\n<\/ul>\n\n\n<p>\n\n\n  Individuals wishing to learn more about this program should contact the nearest DSS Long Term\u00a0Care unit.\n\n\n<\/p>\n\n\n<h2>\n\n\n  Developmental Disabilities Home and Community Based Waiver Program:\n\n\n<\/h2>\n\n\n<p>\n\n\n  This home and community based waiver program is administered by the <a href=\"\/ddds\/\">Division of Developmental\n\n\n  Disabilities Services (DDDS)<\/a> .
