An application can be initiated by contacting Central Intake at (302) 255-9800. Minimal information is needed to start the application process such as name, address, phone number, and date of birth.\r\n<\/p>\r\n\r\n\r\n\r\n\r\n\r\n\r\n<p>\r\n\r\n  In order to determine eligibility, a copy of the most recent eye examination information must be provided.
