Community Health Centers
Delaware has community health centers located within each county. These centers are well equipped to respond to medical needs and provide a medical home for a variety of people in almost all income levels. The health centers see insured, under-insured and uninsured patients. Most centers bill their patients on a sliding fee scale, based on the patient's income. These centers are not operated by the Division of Public Health but are listed here for your convenience.

Provides primary care services to children at ten locations throughout the state. In New Castle County: Philadelphia Pike, St. Francis, Jessup Street, Penn Mart, Peoples Plaza and Rockland Road. They provide general exams, blood pressure screenings, minor illness treatment, women's health services, dermatology, adult/child immunizations and referrals to specialists or clinics. Volunteer physicians provide these services.

Provides a hospital-based comprehensive family health office, including obstetric and newborn care through Tiny Steps, a program for low-income pregnant women, staffed by resident doctors providing health screening, diagnosis, and treatment to patients of all ages. The Saint Clare Outreach Program is a mobile van staffed by a doctor, a registered nurse and one assistant that travels to different locations in Wilmington to serve the homeless and others in need. The Center of Hope, a family health center in Newark, offers health care to migrant workers. The Tiny Steps program is also offered at the Newark location. Provides a wide range of primary and preventive health and social services. Health care services include sick and well care, exams, immunizations, family planning, breast and cervical cancer screening, and health insurance and prescription assistance referrals. Staff are bilingual in English and Spanish. Medicare, Medicaid and sliding fees are available for the insured or underinsured. Day or evening appointments are available.
The Kent Community Health Center and the Mobile Access to Community Health (MATCH) Van are both projects of Delmarva Rural Ministries. The mobile unit provides preventive health education, health screenings, immunizations and prenatal tests to select locations within Kent and Sussex Counties. It also assists people in accessing a medical home. The MATCH van is available to participate in health fairs and other community health events.

Services include pediatric, adolescent and adult medicine preventive programs, diabetes education, immunizations for children, routine school and sports physicals, HIV testing and counseling, prenatal services and mental health and substance abuse counseling. Application assistance is available for Medicaid, Community Healthcare Access Program (CHAP) and other programs. Services are based on sliding scale fees. Staff is fully bilingual in English and Spanish. Translation and transportation services are also available. La Red is Spanish for "The Network."
