About 300 vaccination appointments for those most at risk for H1N1 are still available for this weekend's vaccination clinics in Sussex county. H1N1 vaccination clinics will be conducted by the Division of Public Health, on November 22 at Del Tech-Owens, for pregnant women, any children 6 months to 4 years old, and individuals 5 to 64 years old with certain chronic health conditions who have not yet been able to obtain an immunization. These vaccinations will be given by appointment only. Delawareans in the target groups can call 1-866-408-1899 from 8 a.m. to 4:30 p.m. to make an appointment. After making your appointment(s), print a copy of this Vaccination Consent Form for each person who will be receiving a shot. Fill out and sign the form and remember to take with you to the appointment.
