Apply for Services

An application to DVI for services can be initiated by any individual who is either visually impaired themselves or is aware of a visually impaired individual. An application can be initiated by contacting the Central Intake coordinator at (302) 255-9848. Minimal information is needed to start the application process such as name, address, phone number, and date of birth.

In order to determine eligibility, a copy of the most recent eye examination information must be provided. See Eligibility on the home page for more details.
