Incident Reports

DDDS strives to evaluate the total volume of incidents that occur to the individuals it serves, and understand their severity, regularity, and trending over time. A report is completed annually that details the number of investigated incidents by type and service setting for the prior state fiscal year. DDDS aims to use this information to identify areas of success, and improvement, with the goal of utilizing it to assist in safeguarding individuals served from abuse, neglect,and mistreatment.

The table below displays the total determinations, or final categorizations, of investigated incidents since state fiscal year 19. Only substantiated incidents that were attributed to a breach in policy, protocol, or standard of care are included. It is sometimes necessary for a given incident to be assigned multiple determinations to fully capture what occurred. Please refer to the available Annual Reports for further information.

General Reports

National Core Indicators (NCI) - National Core Indicators (NCI) is a voluntary effort by public developmental disabilities agencies to measure and track their own performance. The core indicators are standard measures used across states to assess the outcomes of services provided to individuals and families. Indicators address key areas of concern including employment, rights, service planning, community inclusion, choice, and health and safety.
