FITNESS FOR DUTY CERTIFICATION FORM

Your treating physician must complete a Fitness For Duty Certification Form prior to the day you return to work. You must present this completed form to your supervisor before the start of your workday. A copy must also be sent to the ACT Case Manager that managed your case during your leave.
