Required Forms
Employees are required to complete a Parental Leave Request Form.
Employees must also provide legal documentation of birth or adoption of a child within 30 days of the birth or adoption or as soon as it is available.
Legal documents include:
- Birth Certificate
- A Birth Certificate is provided by the State’s Office of Vital Records.
- OR Consular Report of Birth Abroad
- A Consular Report of Birth Abroad of a Citizen of the United States of America (CRBA) serves as proof of U.S. citizenship. If your child was born abroad, you should apply for a CRBA by contacting the nearest U.S. embassy or consulate.
- Adoption Order
- An Adoption Order may be accessed via the State’s Office of the Courts.
