How do I apply for benefits through the MWD eligibility pathway?

To apply for MWD, call the Milford State Service Center at Riverwalk to request an application. You can ask for an application to be mailed, faxed, or emailed to you. Find printable versions of the application in multiple languages below.

What do I need to apply?

You must provide your completed application, and you may also be asked to provide the following information if DMMA cannot verify it electronically:

  • Proof of citizenship and identity, or immigration status.
  • Proof of income – a full month’s paystubs, or the latest tax-return if self-employed.
  • Proof of disability. (If you have not been determined disabled, we can assist in initiating a determination for you.)

If my application for MWD is approved, would I be eligible for retroactive coverage?

You may be eligible for retroactive coverage for up to three months prior to your application date! If you received services before you applied for MWD, please let us know when you submit your application.


If my application for MWD is approved, when does my coverage start?

Once we make a determination on your application, information will be sent to you. When your application is approved you are enrolled in a Managed Care Organization (MCO). If you prefer a different MCO you have 90 days to change your enrollment. Your services begin when you are enrolled with an MCO.
