An Injury/Illness Report will need to be filled out to start the process.

  • Click on the link to access the Injury/Illness Report Click here
  • The supervisor submits the Injury/Illness Report to the ACT Case Management team at dhss_dmsfmla@delaware.gov
  • The ACT Case Management team submits the claim to the state's workers' compensation third party administrator, Pennsylvania Management Association (PMA).
