Health Care Relief Fund Deadline Extension

The deadline for both Round 1 and Round 2 recipients of the Health Care Relief Fund (HCRF) grants has been extended to December 31, 2021. Recipients interested in extending the deadline or their grant much complete the below Extension Certification form and return a signed version to DHSS_CaresAct@delaware.gov by no later than October 31, 2021.

A final expenditure report will be due to DHSS in January 2022. More details on required program close-out will follow but grant recipients should be prepared to submit their final expenditure report and corresponding receipts. Unspent funds are required to be reverted to the federal Department of the Treasury.
