Medicaid Waiver for the Elderly and Disabled
| Program/Service description | The Medicaid Waiver for the Elderly and Disabled provides an alternative to nursing home care for eligible older persons and adults with physical disabilities. The program includes services to help a person to continue living in his or her home safely. Nurses and social workers coordinate with participants and their caregivers to develop care plans that help to meet individual needs. The services that are part of this program include: |
