What activities are subject to Certificate of Public Review?
The activities subject to review are : The construction, development or other establishment of a health care facility or the
acquisition of a nonprofit health care facility; Any expenditure by or on behalf of a health care facility in excess of
$5,800,000, or some greater amount; A change in bed capacity of a health care facility which increases the total number of beds
(or distributes beds among various categories, or relocates such beds from 1 physical facility or site to another) by more than
10 beds or more ; The acquisition of major medical equipment, whether or not by a health care facility and whether or not the
acquisition is through a capital expenditure, an operating expense or a donation. The replacement of major medical equipment with

How do I file a Certificate of Public Review application?
Notice of Intent must be submitted at least 30 days but not more than 180 days prior to submitting an application. The Notice of

What are the application fees to file a CPR application?
The application fees to file an application are:
- Less than $500k = $100
- $500k to $999,999 = $750
- $1,000,000 to $4,999,999 = $3,000
- $5,000,000 to $9,999,999 = $7,500
- $10,000,000 and over = $10,000
