The Division of Public Health’s Data Information & Request Process page is designed to centralize all of the various data and statistical reports maintained by the Division, and to streamline the process for health data requests. This page lists the major categories under which data maintained by the Division is classified (i.e.: Diseases and Chronic Conditions, Environmental Health, etc.). Under each major category is a list of data subtopic links. These links take you either directly to a report or table or, to a webpage with more data and information for you to search. The ‘More’ link under each major category, takes you to a separate page listing Quick Facts and/or links to additional data. If you do not find the information you seek by visiting one of the available links, you can make a request for specific data by selecting the ‘Data Request Process’ link near the bottom of each major category page or on the home page (under Data Guidelines). We ask that you review the information under the Data Guidelines section to become familiar with the different types of data requests, and what is considered publically available data. Members of the media seeking additional information must contact the Office of Health and Risk Communication instead of submitting requests via the Data Request Process link.
