Information on replacing food lost as the result of Hurricane Sandy


If you purchased food with your EBT card and that food was destroyed as the result of Hurricane Sandy, the Division of Social Services (DSS) can help. Federal regulations allow DSS to give you more food benefits to help you replace the food you lost.

  • You must sign a sworn statement telling us the value of the food that was destroyed. (DSS cannot replace more than your monthly benefit amount.)
  • You may be required to verify the loss of food.
  • You must swear that the information you are giving us is true and correct.
  • If you intentionally misrepresent the facts you can be prosecuted in a court or referred for an Administrative Disqualification Hearing. If found guilty, you will not be able to get food benefits for one year for the first violation, two years for the second violation, and permanently for the third violation. You can also be charged with perjury for a false claim, fined, or sent to jail by a court.

You can request replacement of lost food due to Hurricane Sandy anytime from October 31, 2012 through November 16, 2012. We will not accept any requests for replacement after close of business on November 16, 2012.

You may make a request for replacement in one of two ways.

  1. You may go to your local food benefit office and complete an affidavit requesting replacement of food benefits.
  2. You may download the affidavit and complete it. Then mail or fax to your local food benefit office.
