American FactFinder American FactFinder provides access to data about the United States, Puerto Rico and the Island Areas. The data in American FactFinder come from several censuses and surveys.
Behavioral Risk Factor Surveillance System(BRFSS) - Prevalence and Trends Data The BRFSS is the nation's premier system of health-related telephone surveys that collect state data about U.S. residents regarding their health-related risk behaviors, chronic health conditions, and use of preventive services.
CDC Community Health Status Indicators (CHSI) CHSI 2015 is an interactive web application that produces health profiles for all 3,143 counties in the United States. Each profile includes key indicators of health outcomes.
CDC FluVaxView Interactive National and state-level influenza vaccination coverage information.
CDC National Center for Health Statistics (NCHS) The NCHS is the nation’s principal health statistics agency, providing data to identify and address health issues. NCHS compiles statistical information to help guide public health and health policy decisions.
CDC WISQARS CDC’s WISQARS™ (Web-based Injury Statistics Query and Reporting System) is an interactive, online database that provides fatal and nonfatal injury, violent death, and cost of injury data from a variety of trusted sources.
CDC WONDER WONDER online databases utilize a rich ad-hoc query system for the analysis of public health data. Reports and other query systems are also available.
The National Survey of Children's Health The National Survey of Children's Health provides rich data on multiple, intersecting aspects of children's lives, including physical and mental health, access to quality health care and the child's family, neighborhood, school, and social context.
US Census Bureau-American Community Survey (ACS) The ACS is a nationwide survey that collects and produces information on demographic, social, economic, and housing characteristics about our nation's population every year.
Youth Risk Behavior Surveillance System (YRBSS) - Youth On-line Data Analysis Tool The YRBSS provides data for priority health risk behaviors that contribute markedly to the leading causes of death, disability, and social problems among youth and adults in the United States.

America's Health Rankings America’s Health Rankings releases two yearly reports, one on the health of the general population in the 50 US states plus a senior report on the population aged 65 and older in each state. Both reports include some health metrics that are stratified by race/ethnicity, gender, age, education, place of residence, and economic status.
County Health Rankings The County Health Rankings measure the health of nearly all counties in the nation and rank them within states. The Rankings are compiled using county-level measures from a variety of national and state data sources. These measures are standardized and combined using scientifically-informed weights.
Delaware Focus Delaware Focus provides data for community health indicators on measurements of social, environmental and economic factors that affect quality of life.
Delaware Health Tracker Delaware Health Tracker brings non-biased data, local resources, and a wealth of information to one, accessible, user-friendly location.
