Delaware Division of Developmental Disabilities (DDDS) Home and Community-Based 1915(c) Lifespan Waiver Amendment

In accordance with the public notice requirements of 42 CFR 441.304 (d1)(e) and (f) and the state public notice requirements of Title 29, Chapter 101 of the Delaware Code, Delaware Health and Social Services (DHSS)/Division of Medicaid and Medical Assistance (DMMA) gives notice related to the amendment of the DDDS HCBS Medicaid Waiver authorized under section 1915(c) of the Social Security Act.

The purpose of this posting is to provide public notice and elicit public input regarding Delaware’s proposed amendment to its current 1915 (c) HCBS Lifespan Waiver that serves individuals with intellectual and developmental disabilities (IDD).

DDDS HCBS 1915(i) SPA Pathways

In accordance with the requirements of 42 U.S.C. §1396a (a)(13) and 42 CFR §447.205, Delaware Health and Social Services (DHSS), Division of Medicaid and Medical Assistance (DMMA) gives notice related to the renewal of Pathways to Employment Home and Community Based State Plan Amendment authorized under 1915 (i) of the Social Security Act.

The purpose of this posting is to provide public notice and elicit public input regarding Delaware’s renewal of the Pathways to Employment program that services individuals with intellectual and developmental disabilities (IDD), visual impairments, autism spectrum disorder, Asperger’s Syndrome and physical disabilities ages fourteen (14) to twenty-five (25) to seek and maintain competitive employment.

DDDS Waiver Fee-for-Service Selective Contracting Program 1915(b)(4)

In accordance with the requirements of 42 U.S.C. §1396a (a)(13) and 42 CFR §447.205, Delaware Health and Social Services (DHSS), Division of Medicaid and Medical Assistance (DMMA) gives notice related to the renewal of an application for a fee-for-service selective contracting waiver under section 1915(b)(4) of the Social Security Act.

The purpose of this posting is to provide public notice and elicit public input regarding Delaware’s renewal of the this waiver which seeks to limit freedom of choice of providers of the Employment Navigation service and to selectively contract a transportation broker to provide the non-medical transportation services to Pathways program participants.

The purpose of this posting is to provide public notice and elicit public input regarding Delaware’s proposed amendment to its current HCBS Waiver that serves individuals with intellectual and developmental disabilities (IDD) to create the Lifespan Waiver.

General Information

Delaware Health and Social Services (DHSS)/Division of Medicaid and Medical Assistance (DMMA) and Division of Developmental Disabilities (DDDS) received approval from the Center for Medicaid Services (CMS) to amend the Home and Community-Based Lifespan Waiver on May 25, 2017 with an effective date of July 1, 2017.  Please click on the documents below for more information.
