Attention Medicaid Participants: Eligibility Renewals Restarted April 1, 2023
The Transition Age Youth program connects youth ages 16-24, living with MH and/or SUD engaged with youth providers into services with adult providers. The TAY team has dedicated case managers who assist current providers/caseworkers engaged with youth clients by providing case management support, specific to client needs, to alleviate gaps in care. If there is a need for Transition Age services, please complete the Transitional Age Youth Brief Screen and send to: DSAMH_TAY@Delaware.gov.
