New Jersey Academy of Family Physicians and Delaware Academy of Family Physicians - NJ/DAFP's healthcare transformation services combine robust, accredited, evidence-based educational content with quality improvement science to guide primary care practices along the journey to population-level, value-based care. Our approach utilizes expert faculty to develop and teach clinically-relevant educational curriculum, coupled with validated quality improvement survey tools, data review, and on-site interviews and observations to achieve the practice's improvement goals. Our team works with practices to develop a customized improvement plan and establish actionable steps that includes educational curriculum, targeted interventions, coaching recommendations, and community resources (i.e., hospitals, specialists, home health agencies, etc.) in order to achieve successful and sustainable outcomes.
